Peoples Health Employee Recognition Program Terms & Conditions

Overview: The Peoples Health Employee Recognition Program is a service of Peoples Health. This program is provided for all eligible employees of Peoples Health. All users of this website agree that their access to and use of this website are subject to the terms and conditions set forth herein and all applicable laws. Any access is undertaken at the user’s own risk. These terms and conditions may change without prior notice.

Eligibility: All active staff level, below Manager level employees (Participants) in good standing with the Company are eligible to give and receive recognition. Active employees, manager-level and above are eligible to give points only.

Inactive employees are not eligible to give, receive or redeem points until they return to active employee status.

Each eligible employee is automatically enrolled in the Employee Recognition program and will use the ID number provided in the initial registration email sent to each participant to gain access to the program.

Changes and Disputes: Peoples Health reserves the right, without prior notice, to discontinue or change the program at any time.

Peoples Health reserves the right to change these terms and conditions at any time.

All questions and/or disputes regarding eligibility for this program, the eligibility of rewards, the availability of rewards or points, the terms and conditions of the program will be resolved by the Human Resources department of Peoples Health, at its sole discretion.

Earning Reward Points: To support and strengthen the Mission and Vision of Peoples Health, employees may be recognized and rewarded with points for exhibiting the following actions, behaviors and values:

Points will be posted to participants’ accounts upon approval of an award. Participant points will not expire.

Peoples Health may, at its discretion, use the recognition program to promote company initiatives and/or events. These initiatives may be modified and/or terminated at any time, without cause or warning.

Rewarding Points: There will be three types of rewards:

Manager Spot Awards-Managers and Directors can award employees a manager spot award which has points associated to it and includes a personalized certificate.
Employee Recognition-Employees can recognize another employee by creating a personalized certificate for the employee.
Employee Nominations-Employees can submit peer-to-peer nominations in which they nominate an employee for a manager spot award; the request goes to the employee’s supervisor who chooses whether or not to issue an award.

Managers will be able to recognize any employee, regardless of department, for a reward. If the employee is in another department, the points will be taken out of the employee’s supervisor’s budget, based on approval of the employee’s supervisor.

Peer to peer recognition must be approved by the recognized employee’s supervisor if points are to be awarded as a part of the recognition. Employees may also recognize their peers without awarding points.

Manager Budgets: Based on the annual budget, each Manager will be given a quarterly budget of points based on the number of employees they supervise and can disperse at their discretion. The quarterly budgets will be determined by the Human Resources department and are subject to change without notice.

Quarterly departmental budgets will expire at the end of each quarter.

Reward Redemption & Deduction of Points: When sufficient points have been earned, participants may redeem rewards from the Online Catalog. Reward Points may only be used to redeem merchandise from the Online Catalog. Points are not redeemable for cash. Peoples Health reserves the right to modify merchandise available in the Reward Points Online Catalog at any time and without notice. Specific merchandise is subject to availability and substitutions may be made based on availability or market conditions.

If an employee is terminated from employment with Peoples Health, any points previously accumulated are automatically forfeited and will be added into a Forfeiture Account to be used on behalf of the Company, at its discretion.

If an employee resigns employment at Peoples Health, any points previously accumulated may be redeemed before the employee’s last day of employment. Any points remaining following the employee’s last day of employment are automatically forfeited and will be added into a Forfeiture Account to be used on behalf of the Company, at its discretion.

Sales Tax: Merchandise redeemed under the program is subject to shipping and handling charges. The shipping and handling charges are included in the point value associated with each item in the Online Catalog and are represented in the overall value of the item.

Income Tax: The Rising STAR program is intended to be a true benefit to employees. Therefore, based on the value of the reward redeemed and a tax rate of 35 percent, Peoples Health will pay a cash bonus at the end of the year to account for additional taxable income. For example, if an employee redeems an award that is valued at $100, the employee will be paid a bonus of $53.84 to account for the adjusted income tax that the employee is responsible for.

Shipping and Handling: Merchandise redeemed under the program is subject to shipping and handling charges. The shipping and handling charges are included in the point value associated with each item in the Online Catalog and are represented in the overall value of the item.

Privacy Notice: This website is housed and operated by Online-Rewards on behalf of Peoples Health The information collected on this website will be used by Peoples Health in connection with the Peoples Health Employee Recognition Program.
